The cornerstone of a successful and comfortable patient/physician relationship is communication. Each member of our staff is here to assist you.
All physicians are board certified or board eligible with extensive training in the field of obstetrics and gynecology. Our Nurse Practitioners, Physician Assistant and Midwife are trained to meet your obstetric and gynecologic needs and are available to all patients to help provide additional access to care.
We encourage you to maintain a relationship with your family physician or internist for non-gynecological health concerns. If you do not have a family physician, we would be happy to provide suggestions or referrals.
Our services include Obstetrics, Gynecology, Gynecology Oncology, Laboratory Services, Ultrasound, Mammography and DEXA Scans.
Our offices have Nurse Practitioners on staff. The Nurse Practitioner is a registered nurse who has completed postgraduate studies in the field of women’s health. She is an important health care team member who works closely with your physician. She may see you for your annual exam, routine OB visit, post partum visit, or various screening exams.
The office telephone is answered daily from 8:30 am to 5:00 pm. Patients are welcome to call the office with questions. To avoid lengthy hold times and potential phone tag, we encourage you to use our online Patient Portal.
Our office visits are by appointment only and are scheduled Monday through Friday. Some physicians offer early morning or evening hours. If interested, please inquire with our scheduling department.
All medical records are confidential. We comply completely with HIPAA guidelines. If you wish, we can provide a copy of your records on CD or paper. We require three business days to complete your request. There may also be a fee associated with your request.
Our fees reflect the level of care you receive as well as the education and experience of the physicians. Please visit our Fees Page for pricing. We accept all major insurances as well as Discover, Master Card and Visa. We accept Medicare assignment. Copayments are required at time of service.
Our billing department can be reached by dialing (260) 432-4400 and following the prompts or online Patient Portal.
At each visit, we will ask to see your insurance card and will file your visit with your insurance company. If your insurance requires a referral from your primary care physician, it is your responsibility to obtain that referral prior to your visit in our office. Make sure the physician you are seeing is in your insurance network and familiarize yourself with their requirements regarding precertification and second opinions. Report any change in your insurance coverage, personal status, name, address, or telephone number, so we can maintain current records.
Regardless of the insurance company, the patient is responsible for any balance not covered by the insurance company. If you have questions, please call (260) 432-4400 or use our online Patient Portal.
